  • The recipient’s condition has improved
  • There’s new support from other household members
  • The social worker determines that fewer hours are necessary

That’s why it’s important to document medical conditions, keep supporting records (like doctor’s notes), and be prepared to clearly explain any changes during the visit.


Tips for Reassessment Preparation

  1. Track Care Tasks: Keep a daily or weekly log of tasks your provider helps with (bathing, dressing, feeding, mobility, etc.).
